Name to be kept on Custom forms.

Hi,

My Wife does not use a surname and it is not in her passport as well. So the visa was applied by keeping given name to the surname and in the first name they have kept 'FNU'.

Now while entering in the USA one has to fill the Custom form.
Will the same name pattern to be continued in that form also?
Please suggest.

Use the name in the same way as in the visa.
