    Hi there your help.
    I am on the process of filing my ds230. Just recently married and my passport is still under my maiden name and i am not planning to change it yet since it would take a month for it to be processed. But i am quite confused to write on Question1 which asks your family name, First name and middle name. Would i need to place my husband surname already? Even though i have not changed my passport name yet? I learned i can still retain my maiden name.

    Question 2 (if married woman, place your maiden name). basically this question asks for my maiden name which is very clear.

    Quite confused.

    PLs help.

    DS230 is not long being used. It has been replaced with online DS-260.

        i am already current my PD is Jan 2007 and last november 2013 they already advised me to apply the ds230. Until now i have not applied yet as i am waiting for my marriage certificate from the Philippines. Since i have complete documents now, i never throught that i should apply now thru online ds260.

          The DS-230 is now "...Diversity Visa and Cuban Family Reunification Parole applications only..."
          The DS-260 can be accessed at: http://travel.state.gov/contenthttps...isa-forms.html

          The DS-260 became the official replacement for the DS-230 several months ago.
