Affidavit in Lieu of Legal Capacity to Contract Marriage

I plan to go to the Philippines next month to marry my Fiancee. I am aware that I need to obtain this affidavit. What paper works should I bring with me when applying for this certificate? How long will it take for me to receive this certificate? I know this was probably the wrong place to ask this question, but I'm really in dyer need to find out. I hope someone can help me out. Thank you
