Are you a U.S. citizen?

If yes, submit form I-130 with proof of your citizenship, copies of birth certs for you and parent, copy of parents' marriage cert, church, school and medical documents with both your names together, photos of you both together, check for $420 to "U.S. Department of Homeland Security."

Depending on your official legal address, you will have to decide which of the two USCIS addressest to send your documents.
