Can H4 dependent travel based on H1 petition

I have my H1B petition valid till March 19 ,2016 and my wife H4 was also valid till March 19,2016. I got my H1B extension(and amendment also) approved for next 3 years.
No extension/amendment applied for H4 for my wife.

1. When i send my passport for stamping the new VISA , can i send my wife's passport also for stamping.
2. Or do I need to go for extension/amendment of H4 for wife separately for stamping H4 visa.
3. Without stamping her H4 visa, can she travel based on my approval notice?
