H4 - Consular Processing..PLEASE HELP!

my boyfriend is 18 and has been living in the states for seven years off of his dads H1..giving him an H4. his parents claim to have been trying for greencard/citizenship for years, but we think something wrong is going on. for the past year and a half he has lived in indiana, his parents moving back and forth from NJ to TX. he just moved back to canada today so he could have a job. i wanted to know if his H4 applies for consular processing. i thought i read that medical history and relationships with us citizens are catagories applying for it, and with him having open heart surgery here, his doctors being here and very close friends living here, would this help? im sorry this was so long, rambling and pointless to you, but if you know anything please let me know. thank you so so much.
