Hi All,

Me and my daughter have interview scheduled in this month. I received letter of appointment from Chennai but they missed to enclose Packet 4.
When contacted they asked me to download Packet 4 from their website.

I am trying to get the paper work ready with the help of this website.

Can anyone clarify on following :

1. For Affidavit of support which form do I download I-134 or I-864 ?
Do I need this for myself and for my daughter or just my daughter ? Its an employment based immigration.

2. What are the fee amounts for the Demand Drafts ? I paid $335 per applicant to NVC.Please let me know the other amounts for DDs.

3. Is IRS-9003 just a one page document with 4 questions?

4. Is Order of documents important ?

Please let me know if I am missing anything thats not on this website or changed
recently.

Any help will be appreciated.

How did your interview go? Which consulate was it in? I have received packet 4 but that didn't include the IRS form you mentioned, for some reason. Can one turn in downloaded forms?
