Can someone please help me with DS2001 form.
My understanding is that we will require to send this along with the certified copy of I-140, I824 receipt and DS230 Part1.
Do we have to just sign the bottom portion of DS2001 form or do we have to check the checkboxes also?.
Any inputs will be greatly appreciated.
