Questions Regarding Packet III

I have a couple of questions regarding Packet III
1) Do I need to put a cover letter when I send the Packet III to consulate. If so how should it be
2) Is this correct? I need to send OF 230 Part I - One for myself and one for my wife(dependant) and OF 169 - only one. Is there any thing else to be included when I send it to Consulate
3) In OF 169 do I need to check Police Certificates(I am from India)

I would appreciate if any one can answer my questions

I am assuming your case is regular CP. For AC-I140 it could be different. Answers are as under:

1) Yes.

It should include the following:

Case Number:* * * *
PA Name: your name
Preference Category: E1/E2/E3...
Priority Date: * * * *
Foreign State Chargeability: * * * * INDIA

Travelling applicants: NAME,DOB,POB
for you and the dependents.
