DS2001(OF-169) - Do we need to send it along with DS230 -I

Hi Guys,
Can any one let me know if we need to send DS 2001 along with DS 230 -I? According to the instructions I received along with packet 3 (where, they specify a list of documents that need to be sent to St. Louis), they did not mention DS 2001. So, should I just send the DS 230 -I along with the cashiers cheque or should I send even DS 2001?
Also they mentioned that I need to send a cashiers cheque for $335(I received my Packet 3 on June 20th). According to immihelp information, I understand that only $260 need to be sent. Is there any increase in the fee structure in recent times?

Yes, send DS-2001.

You need to send $335. That information has changed as of June 1 and we have not had chance to update it yet. Sorry about it. We will do it soon.

What is DS-2001

I have already sent Packet 3 to NVC St. Louis. I dont remember sending DS-2001. What is this form DS-2001?
