    Urgent: Is IRS-9003 form still required for CP

    I just recieved the Interview letter to my US address. They asked me to download packet 4 from chennai consulate site which I did. It has only OF230pt2 and the medical exam cover letter. Am I missing something and what else is required.

    Is IRS-9003 form still required for CP. I believe this was required sometime back.
