PCC Format

Hi,

Can some one send me format of PCC which was
issued from indian embassy here in US.I want
to compare my contents with others.I fill like
they have done some typo in my certificate or
that is how they generally give to everyone.
I'm confused.

I would appreciate if some one would get back
to me on this asap.
