DS-230 Part1 Section 10 & 11

Hi

Can you please post your comments on section 10 & 11 where we have to give the US address intend to live and green card to be mailed. I have given my US address for this coloumn, there was another question stating "Include the name of the who currently lives there" for this question should I fill my NAME or just address is sufficent.

And also in section 13 I have filled my India address for P4 communication.

Could some one direct me on the above concern. Your help is appreciated.

I also have same concerns. I do not see any discussion on this topic. Which address (10,11,13) does consulate uses to mail packet 4.

Item 10 - I have my name and US address
Item 11 - type "same as #10
Item 13 - My US address
This is what my lawyer has filled for me.

how does the consulate know where to send pac IV, if it is to a indian address and one is living in US
