Does anybody know if the dependents have to fill out IRS 9003 as well ?

Yes, If dependents don't have Social Security numbers, they should use TIN (Tax Identification numbers) for this form. Consulate will send one IRS9003 (Yellow) form for each dependent who are going for CP interview.

What is this form for? I didn't see this in my packet 3 or 4, is this something necessary for the interview? Please let me know. Thanks.

This document is mandatory. If you don't have it, download it from the IRS site and print it on yellow paper.

Believe it or not, this form is not on the IRS website. It is, however, on this site under "filleable forms".
