Can someone tell me

1. what to write in #25 for A number? I am on H1-B visa

2. List dates of all prior visits to United States
what will be "To date" if I am currently living in US
Will it be the date I am signing this application?

1.No need o mention A#...leave blank....

2.To Data ....write as "continuing".
