Is it OF169 or DS2001?

I have to send the AC140 with I824 receipt to the New Delhi consulate. Apart from the AC140,I797 and the I824 receipt I need to send the OF169 Form. Is it still OF169 or is it DS2001?I have seen both the forms and they look similar, but which one is to be sent?

I'd sent OF 169 for regular CP case

BTW u can send the forms and
if it requires any correction, u can take along the corrected or additional forms at the time of interview...
