OF 230 Part2 question- Please help

Hi,
Chennai US consulate has sent me an older OF 230 Part2 and need your help to answer the following items

25. ADDRESS(Local)
&nbsp &nbsp &nbsp &nbsp -Is this the address to which the blue dart courier will be sent the next day?

29. PURPOSE IN GOING TO THE UNITED STATES
&nbsp &nbsp &nbsp &nbsp -Mine is an Employment based GC. Should I fill in as to work or employment...
&nbsp &nbsp &nbsp &nbsp Please, let me know what you people have filled in.

30. LENGTH OF INTENDED STAY (if permanently, so state)
&nbsp &nbsp &nbsp &nbsp -I think GC is nothing but permanent residence status. Like to know what other CPer's have filled in.

I don't know which form u r referring to. In the OF-230 Part II that I have it shows the following :

25) Permanent Address in US

29) Have you ever been charged, arrested or convicted of any offense or crime ?

30) Have you ever been refused admission to US at a port of entry ?

Could you just re-visit this form again ?

25. Courier is not sent to this address. You will receive different form at chennai consulate on the day of your interview.
29. Employment.
30. Permanent.

I haven't seen this form yet from consulate. Is this form on the website. Is it not enough if we fill DS-230 (I & II), since that is the latest one.

Padma61, Did you use the same form ? I am getting confused as to which one the consulate will ask. Pl. clarify

Chennai US consulate has sent me an older OF230-Part2 (Revised 4-91) with Packet 4. In the first page they have listed these questions. BTW, the forms available in chennai US consulate website doen't match with the one they have sent with Paket 4.

Looks like other CPer's have used the forms sent by the consulate for the interview. I prefer to do the same. Some where in the packet 4 they have stated, to use the forms sent by the consulate even though they are outdated.

Hand writing is fine. Write in Capital letters.

Either one forms are ok but the forms which are on the web is different from the forms sent by chennai consulate & they do ask additional information like height,weight etc. so it is better to fill-up the forms sent by consulate.
