Where can I get fillable DS 230 part 2(for packet 4)??

Where can I get DS 230 part 2 other than immihelp, which is necessary in packet 4. Because fillable DS 230 part 2 from immihelp is not printing correcly. All alignments are messed up. I have sent immihelp email about this. Do you know any other links for this form. I tried to search it but couldn't find it. I see everywhere OF 230 part 2 which is now obsolete.

I printed DS230 P2 from immihelp with no problem.
I used Acrobat reader 4/IE/Win2k on HP LJ 4Si

Thank for reply. Perticularly second page have all boxes mis aligned. I am using Acrobate reader 4.05,IE 5.01/windows NT and Lexmark laser printer. I tried IE as well as netscape but the same result. Not sure why?? I didn't have any problem with DS 230 Part 1. Please send me the attachment if you find one at [email protected] I appreciate your help.

I just printed them and found everything fine. Windows Me, Acrobat 4.0, Lexmark inkjet printer.
