Birth certificate affidavit format for CP Help.

I obtained birth certificate affidavits from my parents. I have got 2 affidavits one from my father and one from my mother. The affidavits are executed individually each stating that one is my father and the other as my mother. Is this OK?. I read that the affidavits must have the names of both the parents. Please help. BTW I will do CP in Montreal and is it a must that the affidavit must contain the Maiden name of the mother.

No need for mother's maiden name.

Affidavit itself can be signed by one parent. But in the text, it should have the names of person and his/her both parents.
