Vaccinations for CP

I have already had vaccinations during my i-485 medical exam. What kind of proof I have
to produce during CP medical exam that I had vaccinations before?

vaccinations

you need to get the immunization record from the place you got your 485 medicals done.

This is a small card which lists all the immunizations and the doc's office needs to fill the date on which the shot was administered.
