Rejection at Consular Processing

What are the Possibilities that an IV gets rejected?

What happens to the Non Immigration Visa. I mean if somebody has a H1B Visa and his/her Immigration Visa gets rejected. Is it possible for him to come back on H1 Visa??

Any Inputs will be of a great help.
