Urgent, please help - Police Clearance from UK

I have seen some posts on this board on the same topic but I could not find it now.
Could someone who actually got a polica clearance from the UK please send me the email / Mailing address / Phone number. Also, I will appreciate if you could also tell me what documents / information do I need to supply.

I got a Police Certficate from UK reecently. I applied in July last week and got in Septmeber 1st week.It took aproxmately 40days.I don't have the phone number with me now. but what you can do is, find the UK consulate(in US)phone number on the internet and call them up. They will ask you what part of UK you lived and give you a phone number to call. You just have to call that UK phone number and they will send you a form. Just fill it in and send it.
You will also have to send a draft for 10 pounds sterling.

When I called up the local police station in Lewisham, they said that since I do not live in the UK now, they can't do it. Do you know the form number that you had to fill in. I appreciate your help in this matter.

I lived in Winchester, Hampshire. I called them up and said that I stayed there and I am in USA now and I need a Police Clearence Cert. She said she will send me a form.
On top of the form "Data protection Act 1984" is written and also on the side it says D3/SA 885100.I don't know if this is the form number.
On the top of my form it clearly says "Hampshire Constabulary".
