For the question "If not a native born US citizen"
- If lawfully admitted permanent resident dive "A" number

How am I supposed to answer this question since I am here on H1B?

For the question where it says "If previously filed a prtition with the INS" what am I supposed to answer. I had filed my I140 with my wife as dependant. So do I answer "yes" to this question ?
