ON FORM OF-230 part 1 under section 21, do I need to provide the residence information like I resided at "X" and on "Y" visa type and "123" no for exapmle I was on H4 visa from Feb'98-May'98 at Minneapolis and from June'98 to Dec'99 I was in minneapolis on H1B then I moved rochester with same visa type and No and so forth. Help in this regard greatly appreciated. Thanks.
