I have interview on March 23. I sent an authorization letter to my brother. He will goto consulate next week to pick up my P4.

Is there any sample authorization form available? Also Can i send a embassy a fax or email saying to redirect my Packet 4 to indian address? Have any one tried it.

1. Fax this letter to the person who is going to collect Packet IV. The person can take this letter along with a photo ID to the Chennai consulate (Tues - Fri - 3.00 to 3.30 pm only) and collect the Packet IV.

Medical exam

Surima medical center is not accepting appointments till august. I took an appointment with Dr. Usha Sriram. To my knowledge we do not need appointments for lister labs. Do we?

My interview is schedule for March 21st at 8 AM. Did you receive packet IV at home address in India or did your lawyer at US receive it.

If i need to send in an authoraization letter for some one to collect, what is the format, please do let me know.

Can someone give me a list of doctors for the medical exam? With whom have you fixed appointments? Which doctors were recommended?
