Congratulations! I have my interview in March and wanted to know where you got your medicals done and photographs taken. Did you have to schedule it in advance and how long did it take to get your medical reports and photographs?
    thanks in advance

        Delcp,

Got my medical done with Dr. K.P. Jain at south extension. My father called him in advance for an appointment. However, I think you can just walk in. He has blood test lab in his clinic so you do not have to go to other place. For X-Rays you will have to go to another hospital. We got medical reports in two days. Also, the MMR shot gave me some good grief and Dr. Jain did not tell me that it can cause pain later. I had to call another doctor to find out what I can do about the pain. I could not even lift my hand for 3-4 days.

For photographs, I went to a place KD-169 Pitampura. I do not remember the stuodios name. They do digital pictures and that was good as they could adjust the size on computer.

One more thing, even though you have already sent your Packet 3 for OF-230 part 1, take another copy with you as they asked me for another copy of part 1.

        All the best.

Thanks Mihika. Hope your hand is fine now. I have immunization records to show that I have already taken MMR and DT shots. Do you think that would be acceptable to the doctors at Delhi? Also, did they ask for IRS9003 or a police clearance certificate? Would appreciate your answer.
          thanks once again

Yeah, take your immunization records and doctor will accept them. I took IRS 9003 form with me but they did not ask for it. However, do take it with you. I did not take sailing permit and in my opinion do not waste your time on that.

If you have lived only in US and India you do not need Police certificate, otherwise you may. In my case I lived in UK and did get police certificate from UK. But the lady at the window did not ask for it and neither I gave it to her. Did not cause any problem. However, if you have lived in any other country outside US and India check if you need police certificates. Do take them with you.

              Hi Mihika,
good news and good work . Any way you have crossed the river and might be feeling relaxed.
I have a question regarding "Affidavit Of Support". so for this which form do we get while we get P4 from Delhi consulate. I am waiting for my P4 and in case I don't get it here in U.S., I want to prepare using downloaded fillabale forms.
              I am confused between I-134 and I-864 i.e.which one.
Secondly , Hihg School Certificate should be enough for Birth Certificate or we shuld take some other docs.
Your help is really appreciable as you are helping so many people.
              Thanks a lot!

                Inforyou,

I am not sure if you are Employment based or family based.

For family based immigration you need to take I-864 or what ever the number is.

For employment based you do not need to take any of them. What you need is a letter from your employer that you are employed with them with the current salary and job description (take format from immihelp). However, I took I-134 for my wife and son stating that I will be supporting them. My attorney said it is not required. At consulate they did not ask for it.

For date of birth certificate, I am not sure if the high school certificate will work. You need to get a BC from registrar of birth and death from the the place of birth. Nowadays, you can get that by providing an affidavit from your parents if the birth is not already registered. I did that for my wife and they accepted that BC. The P4 states that if are born after 1969 then you must obtain a BC from the registrar of birth and deaths. If
