question on packet 4

the chennai consulate sent an invitation for the cp appointment but did not include packet 4. in the invitation
letter they make a mention of medical exam instructions but none were included.

can somebody who has completed the medical exams recently let me know the details of this. thanks.

My friend you have to either go in person (Before interview) and get a copy of all the documents & forms or send a fax to somewho you authorize to go in person to the consulate to get the entire set of forms that are necessary.

Note: They never mail the entire list to the US, they send only the appointment letter, so make arrangments.

The counter is open between 3:30 to 4:00 Tue thru Fri
