Which passport copies to include in NVC document submission?

Hi,


I have received the welcome letter from the NVC and finished IV fee payment and DS-260. Next, I need to submit the documents to the NVC and have a few questions -


1) Should I send copies of only the latest passport or all previous (expired) passports?


2) Should I include all pages of the passport(s) including the blank pages?


3) Should the photocopies in color or black and white?
