I have a question please. How do you fill out the G25A form for a spouse. The I-130 is requesting a G25A form for both "you and your spouse". I know how to fill it using my name, but how would I fill it out using my spouse's name? How do I fill it out for my spouse

Do you have a sample form of how I should fill it out for myself and how I should fill it out for my spouse ?
