I will be petitioning my spouse through consulate process in Tokyo and I'm stuck on part 5 of the I-130.
I did petition my ex-wife more than 13 years ago via adjustment in the US, but I can't hardly remember the filing date and where it was approved.
1.) What can I fill in for the dates filed and location? It was too long ago, I can't remember the days, yet alone the month. I think the online I-130 is looking for a mm/dd/yyyy format.
Also, I think the closest field office where we applied was a bit further but I can't recall, because the uscis field offices near my previous residence today are fairly new and were not around 13 years ago.
2.) Also, would I need to plug in her maiden name(maybe her current?) as her family name, now that we have been divorced for many years? Or do they expect me to fill out names that we're used during that time period.
