Re-Issue of expired Indian passport

Hi,
My father had a passport several years before (passport expired in 1989). He had his surname as "RAMASAMY CHETTIAR" in the passport. I am applying new passport for him now with surname as "RAMASAMY" as he has all other proof with surname or father's name as "RAMASAMY". Will the passport officer accept it? or do he need have Annexure "E" ?. or should I leave the surname as its in old passport?
kindly help me before I submit the application form online.

If there is no special need then just put surname as it was in the old passport. Things will be much smoother.
