when i got visa last time in 2008, i got my passport cancelled (filled one page form at that time i remember). it has stamps just like this passport mentioned here.

Do i need surrender certificate? when i was filling visa application this time around, i read something like this in instruction(of visa application about certificate).. if you have "Cancelled" stamp, you need surrender certificate. if you have "Cancelled" stamp with reason (acquired US Citizenship), surrender certificate is not required. can someone confirm it?
I tried emailing washington branch but no reply in 4 days.
