minor passort renewal-full name

Hi,

In the verification of nationality status form they are asking full name of the applicant,full name of father and mother. Is it enough to write just the given name? or Do i need to write both given name and surname as full name?

Both first name (given name) and last name (surname) constitute a full name. Provide the names as per the passport.
