One PDF related to indian passport needs to be corrected on immihelp.com

Dear immihelp,

This is with regard to Nationality status verifcation form available in immihelp.com for indian passport.

I am no able to update Date and place of birth field available in this form.Its copying the same Information in all the three fields.Below is the example

I am unable to edit the fields. Hence would request you to do the necessary changes if necessary and provide the corrected form on immihelp.com. This will be helpful to all in future those who are applying for Indian passport.
