Hello Friends,
I am in USA in H1 Visa Extension and My wife is on H4 Visa
We are blessed with a baby boy and we got his passport..
We need to apply PIO Card for him, Need your help to verify the below checklist, so that we will not miss any requirement document.
1. Travisa Order Form & Cheklist
2. A. PIO Application Form- Copy-1
B. PIO Application Form- Copy-2
3. A. Kid’s Original US Passport
B. Kid’s Passport one Copy- of Information Page
C. Kid’s Passport one Copy- of Page 25, 26 & 27 (As it is 28 Page Passport)
4. Four Passport Size Photos (2x2) I hope we can use the same type of photo, which we used for his passport.
5. Kid’s Left Thumb Print in Blue ink (in Signature boxes) and Father & mother’s signature in respective field.
6. One Copy of Kid’s Birth Certificate Whether Original Birth Certificates is also required?
7. A. Father’s Passport one copy whether all Pages OR Information Pages & Visa Page
B. Mother’s Passport one copy whether all Pages OR Information Pages & Visa Page
C. Kid’s Birth Certificate one copy
8. N/A for us
9. N/A for us
10. A. Father’s Proof of Address- one copy of Driving License & Electricity Bill
B. Mother’s Proof of Address – one copy – Lease Agreement Copy
** Whether above Proof of Address Copy will work, or do I need to arrange anything else (like Mother’s Driving License/ State Id Currently She does not have)

14. Whether do we need to submit any India Address Proof (In my Passport, I have one typo in India zipcode, which has not been corrected yet)
15. Money Order of $248.00
a. Consular Fee- $188.00
b. Service Fee- $12.00
c. Inbound Shipping Fee- $24.00
d. Outbound Shipping Fee- $24.00
16. As I will be paying for shipping and Fedex Shipping Label has been generated by them, do I still need to send the return envelope with fedex shipping label?
Please suggest if anything need to be changed / added / removed?
