Birth Date Change

Hi,

My original passport has wrong birth year.This is based on my school certificate which had wrong DOB due to clerical errors.Now i would like to correct the DOB in passport. I have received correct birth certificate from municipal authorities now.

Can you please tell me what all are the documents i need to produce to change my DOB in passport. Will the birth certificate suffice?
