Indian Passport renewal in USA - Question

Hi,

I am in USA and planning to renew my Indian passport in USA - The check list has the following which I would like to get clarification for:
1) Notarized copy of proof of valid visa - Is this the VISA stamp page from passport ?
2) Is it a mandate to have spouse details or marriage certificate sent to BFS even if I am not interested in adding spouse name in the passport ? It is indicated in the check list that, If you specified marital status as 'married' in the application then the marriage certificate's notarized copy should be sent along with the application. Is it a must ? Doesn't sound sensible though..

It would be really great to have information on these. Thanks In Advance!
