  • Affidavit Of Loss and Deemed Surrender Certificate

    Hello,

    I need some help regarding renunciation ( Naturalized 10 Years Prior without Indian Passport ). As per the BLS website I need to send 2 copies of Deemed Surrender Certificate and an Affidavit Of Loss (Notarized).

    In the Affidavit Of Loss Form
    Whose Name do I write in section
    " BEFORE ME, a notary public on the day personally appeared________________ known to me to be "
    Applicant or the notary officer ?

    In the Deemed Surrender Certificate Form
    Section 2
    I have/have not been married ( What is this question about ? Is it asking that how long have i been married ? Or I am married or Unmarried ? )

    And at last does the deemed surrender certificate has to be notarized ?

    These are the most stupid forms I have seen so far in the renunciation form i have to give my b-date twice, address twice and name thrice

    1. Yes, it needs to be notarized.
