2013 OCI Checklist
Please review and confirm. Additionally I have 2 questions.
* Can OCI and renunciation be applied together?
* Can we submit 1 cashier check for all the fees i.e. OCI and renunciation ($282.70 + $175)? Also do we need separate checks for each applicant?

1. Complete OCI online application
- One Part A per person that is applying, and
- One Part B per family
2. One (1) passport sized photo. Photo instructions:
3. The OCI application has multiple places for signature. The following sections must be signed correctly: Sample form:
- OCI Part A: Each applicant must sign COMPLETELY inside the box on their Part A form underneath their picture. If any part of the signature for Part A is outside of the box, the application will be rejected.
- OCI Part B: On Part B, it is very important that ALL applicants sign Part B.
4. You must provide a copy of your valid original United States passport along with your application package at the beginning of the OCI request process. Your original US passport will be required only after the OCI card has been received back from Delhi, India. A copy of the following pages is required:
- Copy of the information page. This page contains the picture and basic details of the passport.
- Copy of the endorsement/amendment pages. On US passport books that end with page 24, copy pages 22, 23 and 24. On US passport books that end with page 28, copy pages 25, 26 and 27.
5. Copy of your Naturalization Certificate. The US Naturalization Certificate should state 'Former Nationality' as India.
6. Proof of Indian Origin: Copy of first three and last two pages of the last Indian passport held
7. Most recent Indian passport (original not copy)
8. Two (2) copies of the completed Renunciation Form
9. $175.00 renunciation fee
10. Proof of Address: Photocopy of state issued ID (Driver License or ID)
11. New OCI Card Fees: Money Order or Cashier's Check (Payable to “BLS International USA” )
- Consular Fee: $275.00
- ICWF Fee: $3.00
- Service Fee: $4.70
- Total: $282.70
