Hi , first thanks to everybody helping each other .
Got my us naturalization in aug 2008 , never renounce my indian citizenship .
Lost my indian passport and already filed police report .
Please help with foll issues if you can
1 do I need renunciation deemed certificate or declaration form only
2. Fees for the renunciation penalty will be 250.$ or 182$
3. Do I need to send the prepaid label only or with the envelope also
4. Money order can be one for the total amount or have to make seperate for fees , penalty .
5. As I have lost my indian passport , for proof of origin of India , my indian date of birth certificate will do or any supporting documents are required
