I am honestly confused with the BLS website. I am planning to apply by mail (to NY office of BLS) for OCI and Renunciation for myself and my wife. We both became US citizens in Jul 2013.
From all the forums and the BLS India finally I was able to make this list. If some of you who have applied to BLS can comment on this checklist I will really appreciate.
MY ENVELOPE (pasted with Shipping labels for myself and my wife which are generated through BLS order form) to BLS NY will contain the following in the order :
**** **** **** **** **** **** **** **** **** **** **** **** **** **** **** ****
1. PART B (SIGNED)
2. ONE BLS ORDER FORM for Myself (OCI + RENUNCIATION)
3. Documents for Myself (2 sets for all documents except the Cashier's check) :
a. PART A (WITH 50 mm X 50 mm photo affixed)
b. CASHIER’S CHECK in Favor of “BLS International USA” for $462.40
c. COPY OF US PASSPORT (Only certain pages as per BLS website)
d. COPY OF US NATURALIZATION CERTIFICATE
e. COPY OF INDIAN PASSPORT (First 3 and Last 2)
Following documents (f to i) since applying for RENUNCIATION together with OCI

f. ORIGINAL INDIAN PASSPORT (LATEST ONLY)
g. COPY OF US NATURALIZATION CERTIFICATE
h. 2 RENUNICATION FORMS COMPLETED AND SIGNED
i. COPY OF INDIAN PASSPORT (First 3 and Last 2)

k. COPY OF DRIVER’S LICENSE
4. ONE BLS ORDER FORM for my wife (OCI + RENUNCIATION)
5. Documents for my wife (2 sets for all documents except the Cashier's check)
*** All same documents as for me in #2 plus the NAME CHANGE COURT ORDER for her
6. 1 PREPAID RETURN ENVELOPE FOR OCI
7. 1 PREPAID RETURN ENVELOPE FOR RENUNCIATION

# Can some please comment if I have missed something or if everything looks in order.
# Can # 6 and # 7 just be shipping labels or should they be actual envelopes?
# Should all these copies be self attested ?
