Passport Renewal hold due to change in signature

Hi,

I applied for Passport renewal for my wife. They put hold saying "Application has been put into pending. application is missing a noatrzied self written letter explaining the change in your signature".

I did not see the instructions/template anywhere to provide notarized copy for change of signature. Could you please anyone provide the template if you come across this situation.
