Need help - OCI re-issue for a minor on a new passport

Hi all

I am in the process of applying for a re-issuance of OCI on my children's new passport.

Can someone please answer the following questions that I have:-

1) Does any of the documents that we send along with the application needs to be notarized ?

2) if yes, then which all documents needs to be notarized ?

3) I am applying for both of my kids. So Should their respective documents be in 2 separate envelopes ?
