Split of Name on Indian Passport

Hi,

I just have my Fist Name Printed on my Passport and the Last Name is left Blank. Because of Which I am going through too many problems. Now I have decided to Split my First Name into First and Last Name going to SFO jurisdiction.

So My question is..

while filling out the forms like BLS Order Form, online Passport Application Form and Nationality Verification Form, Should I give the Name as it is in the current Passport (First Name : AB and Last Name: Blank) or should I mention the Name which I wanted to be like in Future passport (First Name: A and Last name: B).

Please help me out if anybody has gone through the same situation and resolved it in BLS.

Any chance you got this done? If you got your name successfully split, then which form did you use for Sworn Affidavit ? and did you have witnesses signatures?
