Confusion with Maiden name

  • Confusion with Maiden name

    HI,

    Our son was born in USA in month of FEB. I have to apply for long form of birth certificate, US passport and PIO.

    My wife's passport before marriage did not have surname ( field was left blank). After marriage she had received new passport with my surname and it was mentioned on one of the page what her previous name was.

    while applying for my childs birth certificate what should i mention as Maiden name of my wife ? as her old passport did not have any surname at all before marriage.

    Name before Marriage: Reena alias Reenu ( all of it is First/Given name, no surname, we got rid of Alias while we applied for her passport after marriage)
    Name after Marriage: Reena YYYY X ( first name: Reena, last name: YYYY ( chose not to disclose for privacy reasons))

    No one ever asked us to prove my wife's maiden name. We applied for and got passports and other services for three kids in New York state. We typically show our drivers license which has my wife's current surname (which is the same as mine) and no questions were ever asked. The birth certificate will also list your full name.

    Don't you have any other proof of her maiden name? For example, marriage certificate or school certificate will have them. If so, use the name mentioned in those documents in case you need to prove that at any point in the future.

      My wife has only passport as her proof. No DL since she does not drive.
      However marriage certificate has her maiden name , again without her previous surname.

      my concern was , before marriage my wife did not have surname in any of her IDs.

      If i were to provide maiden name for applying birth certificate, what do i do. since she did not have any surname on her previous ID.
      Her current passport has my surname.

        Her current passport has my surname.
        One option is to use her current surname as maiden name as she did not have any surname prior to this. But you should check this with the authorities first. Your state should have a helpline to answer questions.
