  • PIO Card Minor Signatures on how many pages?

    I read this instruction (point 4.2) on BLS website:
    "PIO applications for minors should contain both parent's signatures, on the 2nd page only. Minors do not sign the second page of the application.

    The 2nd page of the application must be duly notarized with a notary stamp and seal, if the application is being mailed to a BLS Center, or both parents are unable to be present while submitting the application in person. Applications without both parent's signatures will not be accepted."

    There is no place to sign on the second page! I ma totally confused, we should sign on third page as well as Second page? Please help!

    PIO Card Minor Signatures on how many pages?

    BLS website instructions are awful. I've had my share of frustration and confusion on these instructions. What I did for my child PIO application.

    Page 1 - Thumb impression of my child, inside the box provided under photo
    Page 2 - No signatures or thumb impression
    Page 3 - My signature under "Signature of Applicant"
    Page 3 - Thumb impression of my child inside the two boxes "Two Specimen Signature of the Applicant"
    Page 3 - My signature and my spouse respectively next to the Signature columns in "Name of the Applicants Father" and "Name of the Applicants Mother".

    Hope this helps.

      There is only one(1) page for the form. Can you please tell me where did you download the file that has three(3) pages?
