OCI Card Help Needed/ Settled in Bangalore

We are US citizens with OCI card settled in Bangalore since last 5 years. My daughter has been reissued a new US passport and according to new rules she has to apply for a reissue of OCI.
I will be turning 50 next year so I will have to reapply for reissue of OCI also.

1. where can I apply for OCI since we are settled in Bangalore.
2. I do not have my latest indian passport. I have only copy of passport page with my photograph and where my old passport said cancelled. I became a citizen in 2006.
3. My wife does not have her indian passport also since she become US citizen in 1988.


what process should we follow to get OCI reissue.

I found out that I can apply in FRRO office Bangalore.
regarding the lost/misplaced indian passports they asked me to give an affidavit to that cause.
