OCI - Requirement - Minor Application

Few Question about OCI Application for Minor

1. Part B - Signature column for applicant ( Who signs here or Should we put thumb impression of minor just like we do for part A )

2. How many copies of the application and documents (green card, passport, license) do we need to send.

3. All copies and All pages needs to be self attested?
