I am applying for myself and my adult children. I have a few questions that the BLS hasn't been very helpful about.

1. If I am sending all 3 applications in the same envelope, do I need 3 separate BLS order forms?
2. In the order form, I clicked on the Fedex link and paid the $15. I got an e-mail but no waybill or airbill number. the BLS order forum wants me to enter a waybill number. How do I proceed?
3. Does the SFO office want me to send 2 copies of everything? This was mentioned rather casually and as an aside in their instructions.

Please treat each application as a separate one. Especially for adult children.
Thus have a separate BLS form.
Also the FedEx payment is per person so you need to pay per order form. I still would recommend prepaid label over theirs. It does not send any airbill no, but BLS seems to send passports through FedEx anyways.

For your third question, yes send a second package per person.
That would mean two set of documents and application with photo per person

I applied to the SFO office of BLS. I have an acknowledgement from them saying that they are returning my daughter's application because it does not have copies of the father's and mother's Indian passports. This is clearly nonsense since I send copies of the 1st 2 pages and the endorsement pages of both my wife's and my last Indian passports as required. Anyway, maybe it is all for the best since the vendor for visa processing is chaning on 5/20.
