I had applied for renunciation from NY consulate. I received the cancelled passport and surrender certificate on time. I noticed that I had filled up only the upper part of renunciation form. In Bottom part( in a box with title Surrender certificate), I did not put my name and country of citizenship thinking that it would be filled by consulate as it seemed to be for official use. Now, I realize that consulate has put date, stamp and signature in surrender certificate part, but did not put my name and country of citizenship. That makes me feel as surrender certificate is not properly completed by consulate. Though my name and all other details are present in upper part of form, I wonder
1. Is it OK to keep certificate in present form? or
2. Should I go ahead and put my name and country myself?
